COVID-19 Patient Screening for Home Visits

    All home visit patients are being contacted by Growth prior to a scheduled visit in order to screen them for possible infection or exposure to coronavirus (COVID-19).  If we are unable to reach the patient or caregiver prior to the scheduled visit, we screen them prior to initiating care in the home.

Screening Questions:

  1. Are you or a member of your household experiencing a fever, cough or shortness of breath?  

  2. Have you or a member of your household had close contact with anyone diagnosed with COVID-19 or recently traveled from a country identified as high risk for COVID-19 outbreaks in the past 14 days?

 

Screening Results for Home Visits:

    If the patient answers yes to both questions by telephone screening, contact the prescriber to report patient’s symptoms and make arrangements for further patient evaluation as ordered by the prescriber.  Ensure that your nurse wears an N-95 mask, eye protection, gown and gloves before entering the home.  Notify your Coram Clinical Trials (CCT) project manager if you need to reschedule the patient’s visit.

 

    If the patient answers yes to the symptoms described but no to the travel risk factors, ensure that your nurse is aware of the need to wear an N-95 mask, gown, eye protection and gloves before entering the home. Notify the prescriber of the patient’s symptoms, for determination of whether testing should be initiated.

 

    If the patient answers yes to travel risk factors, but no to the symptoms described, ensure that your nurse who will be making the home visit is aware of the need to wear an N-95 mask.
